A tire shop can break down the tires and move the sensors although it is usually better to go with new ones in the new wheels. All they do is push the tire bead down, reach in, grab the old sensor, and remove it. Then they push the bead down on the new wheel/tire combo and put the sensor in.
Almost as easy as changing a standard valve stem. No need to rebalance the wheel since they don't move the tire.
